Social Worker

The Short-term assessment and reablement team in the high peak are looking to recruit a qualified and experienced social worker to join our friendly and supportive team. 

The post is for 22 hours and 12 minutes working across all areas of the High Peak.

This post includes all new work coming into adult care and undertaking an assessment for what care and support is needed for the adult.  Working with all adults from 18 years above.

An understanding and experience of safeguarding is also essential as well as experience of undertaking capacity assessments in accordance with the Mental Capacity Act 2005.

The role includes engaging short term home care support, general social work support, Mental capacity assessments and undertaking safeguarding enquires.

This is a busy team with a variety of work being undertaken.

High peak covers Glossop, the centre of High peak and the South of High peak including Buxton.

The successful applicant will be able to demonstrate the necessary skills, knowledge, and experience to undertake this role to provide a safe, accountable, cost effective and efficient service to our clients which helps them to achieve their outcomes.

Key responsibilities will include carrying out Care Act 2014 assessments and reviews for individuals with complex needs as well as arranging the provision of services to meet eligible needs. It is expected that you will be committed to working in a person centred and strengths-based way with clients and their families to maintain and maximise their independence.
